It all depends on who is selling the service and what kind of service.
ADSL is normally flat rate, but sometimes providers use a 'fair use
policy' (excessive use is 'punished') or a monthly bandwidth limit. KPN
just providers the copper wire and some basic DSL infrastructure. IP
transit is delivered by one of the many internet providers.
Dial-up lines are usually charged per minute/second, although e.g. Demon
can deliver flat fee ISDN lines for over 200 euros per month.
